What Does this MRI Scan Result Mean?

Good day..iam 41 years old. I have arm and shoulder pain and may doctor request for MRI the result is Cervical Spodylogenerative changes C3/4 diffuse posterior bulge. C4/5 broad base central and biforaminal disk herniation with bilateral foraminal compromise C5/6 central posterior disk protrusion. My neuro doctor said i need surgery but i refuse to do it.he recommend physiotherapy..after treatment it relieves the pain temporary but still their is pain in my arm everyday pain because of my work as a dental nurse..my question is do i need to do surgery? Some says someday i can not move my arm anymore..

I really appreciate your concern, if the medical management is not cure the disease then surgery is the second option can be tried but prior to this better to have second opinion from orthopedic and neurophysician hope this information helps.
